Pediatric Care
GEORGIA PEDIATRIC PROGRAM
The Georgia pediatric program, more commonly known as GAPP, is a program for parents who need help with their child’s daily living needs to include feeding, bathing, as well as things such as getting dressed, Trach care, G-Tube care, and even assistance with taking medications.
The In-Home Skilled Nursing program is intended to help serve qualified active members based on medical needs for children under the age of 20 years and 11 months.
Our overall goal here at Horizon Care Partners is to ensure you and your loved one is obtaining the necessary support and care needed. As an approved GAPP home health provider, children and young adults under the age of 21 is eligible for free in home care.
